So, here it is, If your issue isn’t listed here, I either forgot about it or I simply don’t have any advice. Please leave a comment if you’re struggling with something you don’t see here. And I know very little about the concerns of uncircumcised men.

This is almost certainly your ball gap. You need a larger one.
The trick is you don’t want to solve it with a larger ring. You want to solve it with a curved ring (e.g., KINK3D), a larger angle between the cage and ring (Cherry Keeper), or a larger junction (CB-X style devices or how Mature Metal would handle it).
According to this comment by u/tdfun, many if not all, plastic cages can have their rings bent in boiling water. You would want to bend the bottom of the ring away from the cage. Don’t burn yourself – tests have shown boiling water is hot.

Your ring is likely too large. Many people don’t feel safe wearing what I would consider a properly sized ring. Provided you aren’t an extreme grower girth-wise, you should be able to wear a heavy steel cage without a strap. I grow 15 mm girth-wise and I can do so. If you’re uncomfortable with the idea of wearing a ring sized how I would size mine, yeah, you’ll need a strap.
My ring size is 5 mm smaller than my erect girth. It absolutely restricts blood flow. I am confident it isn’t an issue is that’s what cock rings do and critically, I return to flaccid without climaxing. But it’s comfortable and nocturnal erections do not wake me up at night.
Wearing just the ring, a full-mast erection should be noticeably harder and veiny. It should be tight but not uncomfortable. You must be able to easily return to flaccid without climaxing. It may take a bit longer but it shouldn’t take significantly longer than without the ring.
If smaller rings have caused a burning scortum, see the above issue and address that with a properly sized ring.

It’s very likely your cage is too long. Bend over and look between your legs. Is the head of your penis no longer against the end of the cage? You want the cage’s length to be short enough that the tip of your penis is still against the end of the cage when you bend over.

I’m having ball escape
First of all, don’t go chasing a smaller ball gap. The ball gap should not be used to prevent ball escape, that’s the ring’s job. And your ring is too big. See My cage is constantly slipping down unless I wear a strap.
I can’t find a ring large enough
I measured and divided by 3.14 and nothing I can’t find anything large enough.
You can’t measure for ring size. You just can’t. Get a device with four or more rings between 40 and 54 mm (54 is really big by the way).

I can’t get my balls through the ring
There’s a trick to putting the ring on. First of all, if you’re extremely high and tight (very little loose scrotum), it’s going to be a challenge. Go take a warm bath to loosen up.
To get the ring on, place the ring under your balls and then pull the scrotum under the larger testicle through the ring. The testicle should drop right through. Repeat with the other but you’ll need to kind of nudge it through from the top to get it through. Once the balls are through, then push the penis through. If your ring is sized correctly (it most likely will be too large), you’ll find it more comfortable to put a bit of saliva on the top of your shaft.
In my flat cage, it hurts when I urinate or climax
Stop using a flat cage. You’ve urethra is a garden hose with a kink in it – get it, a kink. It’s always going to hurt (and may be dangerous) while you’re wearing that cage.
My balls feel slightly cooler with the ring on
They likely aren’t, not outside of what you’d expect with them being pulled away from the body. If you can get a finger between your ring and cage it is very unlikely your ring is too small.
I’ve tried everything and I still can’t get comfortable in a cage
Fortunately, there’s an out. Try a shaft-only device. Currently, I will recommend The Grip as it doesn’t require a piercing or tetherspout. Unfortunately, it is unlikely to be comfortable for uncircumcised mend.

This will be the occasion to thank you for all the ressources and help you gave all around. Goldmine.
One issue I met not listed is that you can burn (in the literal meaning) your scrotum in the 6 o'clock area (where the ring meets the skin in a line).
My own diagnosis is: slightly too big of a ring, unprocessed Cherry Keeper ring and (no surprise there) a ball gap.
But I also cumulate the edge cases: girthy, uncircumcised and a slightly too small of a cage in length (personal preference) which when you add them, cause an excess of skin getting out of the cage's entrance (not helping for the ball gap either).
For some time I thought it was caused by the skin folding/pinching in this area when the ring moves a bit (and thus thinking the ring is too small) but it is bound to happen because of the ring size versus the diameter of the area involved.
My current best guess is the addition of the roughness of the ring and the movements of it caused by moving around or natural ... excitation. Ring itself alone wasn't exactly comfortable until I used sand paper on it but there is no way I can sleep with the cage on (ball gap). It's fine during the day though.
Lesson learned: unprocessed is very bad. Mistakes make us stronger.
Ring current size makes me barely veiny when I am ... At the peak of excitation and I can get in and out of it pretty easily without lubrication. So I assume going slightly smaller might help with the movement/scratching. Lot of assumptions there. And not forgetting about a smooth finish.
I'll also try a slightly longer cage to see if it helps with the extra folded skin at the cage's entrance and the ring pressure on it.
I do not think it's the dye because the shaft presented no issue. But we'll also do without next purchase. Troubleshooting and experimenting around can be fun.

On a side note in the "I keep having to re-align every time I go to pee" part you mention in the 1st sentence that "It’s very likely your cage is too short" shouldn't it be "too long" instead of "too short"?
